Brookside GP practice was named winner of the Health Service Journal Digital Award.
Their project was entitled: Maximising Capacity and Continuity in Primary Care
The project used patient segmentation (created by Connected Care using the John Hopkins Adapted Clinical Groups) and Anima online consultation software to enable the use of a Patient Health Teams Model. The model of care splits patients into the usual teams, including a partner, associate, physician’s associate, social prescriber and patient services liaison (member of our reception team). By using these tools, organisers say they can improve continuity, reduce clinician admin and make best use of ARRS (additional roles reimbursement scheme) roles.
